Application for Fellowship Checklist for U.S. & Canada

Applications are now being accepted for 2025.  (The application deadline is December 1, 2024)

Apply now and you will be considered for induction into the College as a Fellow in 2025. The application process concludes with the granting of Fellowship each year at the ACS Clinical Congress.

Review the Statements on Principles and the Fellowship Requirements for more information on the ACS expectations of Fellows and the application process.

Please note: If you are NOT certified under the American Board of Medical Specialties (ABMS) and meet one of the following three (3) scenarios, you will need a waiver before applying. Please DO NOT proceed with completing an application at this time and review the waiver instructions .

  • Scenario 1: You are working in the US or Canada but were internationally trained or do not have U.S. or Canadian board certification.
  • Scenario 2: You are a dual-degree oral and maxillofacial surgeon (MD,DDS) without Board certification from the ABMS.
  • Scenario 3: You are a single degree (DDS/DMD) oral and maxillofacial surgeon.

Eligibility Requirements

I have the names of three (3) references, at least two who are active Fellows of the College, along with the chief of surgical services of my primary hospital.

I am certified in my specialty by an American Surgical Specialty Board which is a member of the American Board of Medical Specialties, or an American Osteopathic Surgical Specialty Board, or an appropriate specialty certification by the Royal College of Physicians and Surgeons of Canada.

I have practiced surgery for at least one year since the completion of my formal training.

I have been in practice for twelve consecutive months in my current location.

I hold a full and unrestricted license to practice medicine and surgery in the state or province where I practice and know of no adverse actions that would affect the status of my licensure.

I hold a current appointment on the surgical staff of a hospital with no reportable action pending that could adversely affect staff privileges at any hospital.

I practice as a general surgeon or as a surgical specialist within the scope of my specialty.

We request that you be prepared to upload a current Curriculum Vitae (CV) into the application.

Once you submit your application you will receive an email confirmation outlining the next steps in the application process. This will include information on when to submit your Surgical Case List reflecting the type and volume of surgical cases you have performed during the past year, and when your Interview (excludes military) will be conducted with the Committee on Applicants. Application Fee

If you are not a current ACS member (Associate Fellow) you will be required to pay a non-refundable $200 application fee (US) on-line at the end of the application.  Applications will not be processed without payment of the application fee.

ACS Membership

You will become an  Associate Fellow member of the College while your Fellowship application is being processed.  After you are inducted as a Fellow, you will be billed for dues in January to maintain your membership for the year. The FACS designation (Fellow of the American College of Surgeons) can only be used by dues paying ACS Fellows.
