Cancer Program Standards 2012, Version 1.2.1: Ensuring Patient-Centered Care
This manual encompasses the restructured requirements that each program must meet prior to survey and includes the revised and new standards that must be met in order to obtain CoC accreditation.
The manual includes standards that are designed to address patient-centered needs, including mechanisms to address psychosocial distress, manage health care barriers and disparities through a patient navigation process, deliver genetic testing and counseling, and provide for preparation and distribution of a survivorship care plan at the conclusion of treatment. The standards have also been enhanced to increase the focus on evaluating the quality of care and outcomes through the introduction of expected performance levels for accountability and quality improvement measures, as well as an annual study that assesses the appropriateness of patient evaluation and treatment planning.
This publication replaces all previous versions of the
manual, including: Cancer Program Standards 2009: Revised Edition and Cancer
Program, Standards 2012: Ensuring Patient-Centered Care, and Cancer Program
Standards 2012, Version 1.1: Ensuring Patient-Centered Care, and Cancer
Program Standards 2012, Version 1.2:Ensuring Patient-Centered Care. The requirements in this manual are to be implemented January 1, 2012 and should be used to support compliance with CoC standards for accreditation beginning in 2012 and forward.
